The Pulse Paradox of GnRH

GnRH contains one of the most counterintuitive lessons in endocrinology: it's a stimulator of LH and FSH — but only when delivered in pulses. Give it continuously and the pituitary does the opposite: it shuts down gonadotropin release entirely.

That pulse paradox isn't a bug; it's exploited clinically. Long-acting GnRH analogs (leuprolide, triptorelin) are used to suppress testosterone in prostate cancer precisely because continuous signaling = suppression. Native pulsatile GnRH, by contrast, preserves physiological stimulation.
