Survodutide (BI 456906) is an investigational once-weekly dual agonist of GLP-1 and glucagon receptors in development for obesity and for metabolic dysfunction-associated steatohepatitis (MASH). The glucagon component is of particular interest for its potential effects on hepatic fat, and early trials reported strong MASH resolution signals.

Mechanism of action

Survodutide activates GLP-1 and glucagon receptors. The GLP-1 component drives appetite suppression, slowed gastric emptying, and insulinotropic effects; the glucagon component is hypothesized to increase energy expenditure and directly affect hepatic lipid metabolism — the rationale behind its MASH (NASH) development program.
